Case Studies
Case Studies ensure the RFS is
real-life-aligned. The "Briefs" are taken from daily media
stories. Major accident reports are sourced to official websites.
You should read all Briefs (more are supplied each month). As well,
select one major Report each month to review.
Additional Case Studies will be supplied from time to time. You
will be notified by email when that happens, and there will be a
reminder on the Notice Board.
You should enter notes in your Diary on thoughts prompted by Case Studies – observations and ideas as to mishap causation – and recommendations for safer practice in that field or area of practice ("Defences").
The greater the evidence in your Diary – attesting to your ongoing commitment to Red Flag and thereby seeking the highest safety standards you can attain – the more credit worthy your status.
Each Case Study will be accompanied by thoughts on "what went
wrong". The debrief of the case study will be provided one
month after the case study is listed.You should read debriefs after
making your own notes, then compare them with your notes, and record
any areas of strong agreement or dissent.
Whatever their source – and you can begin to compile your own – Case Studies
confirm – over and over again – the continuing, perverse
(unbelievably so!), existence of the most common and virulent accident
cause:
The dogged refusal to accept that something is going
wrong.
